What this document is
This package pairs the City of Plano's 2025 plan document and summary plan description with its active-employee benefits guide for 2026. These are plan and enrollment materials, not an executed services agreement between Plano and a PBM (2025 Plano plan document and SPD; 2026 Plano benefits guide).
The SPD identifies a January 1, 2025 restatement and version V02132025, with a calendar-year plan. What it does tell us
The SPD names Liviniti as the “Prescription Drug Plan Administrator.” The later guide states that employees enrolling in the City's medical plan automatically receive prescription coverage through Liviniti, “the City's pharmacy benefit manager (PBM)” (2025 Plano plan document and SPD; 2026 Plano benefits guide).
These are two distinct, source-dated statements supporting Liviniti's identified pharmacy role. They do not establish the legal name on a signed agreement, the procurement award date, or which negotiated amendments applied during either year (2025 Plano plan document and SPD; 2026 Plano benefits guide).
The 2026 guide also names Healthcare Highways as the default medical plan when no medical plan is elected during open enrollment. That is an enrollment instruction from the guide, not evidence that a medical-plan name should replace Liviniti as the pharmacy administrator (2026 Plano benefits guide).
