What this document is

The acquired 38-page packet contains resolution 2022-R0416, contract 16694, a signed BCBSTX Benefit Program Application effective January 1, 2023, and a pharmacy fee-schedule addendum with pricing periods for 2023–2025 (City's permanent document viewer). The packet is historical evidence; those pricing periods do not prove that the underlying agreement terminated in 2025.

What it does tell us

The application and addendum establish that historical administrative and pharmacy terms were documented, but the scan's pricing tables require image-level verification before extracting numerical guarantees (historical packet). The 2026 benefits guide separately identifies BCBSTX and Prime Therapeutics, without supplying a replacement executed vendor agreement (2026 guide).
