The City Secretary routing slip on page 86 records an effective date of 01/01/2026 and an expiration date of 12/31/2028. Renewal options are options, not exercised terms. The pricing attachments (Attachment B-1 fees, Attachment B-2 pharmacy pricing guarantees and PMPM Target, and the high cost drug schedule) are included in the posted document.
