What a testosterone blood test measures

A testosterone test measures the amount of testosterone in a blood sample. Testosterone may be bound to proteins or unbound. A total testosterone test measures bound and free testosterone and is the most common test. Free and bioavailable tests are less common and may be useful in particular clinical situations. Timing and repeat measurement are part of the process

The laboratory method can affect the result

The Endocrine Society warns that nonstandardized assays can cause the same sample to read low or normal depending on the laboratory and method. A workup looks for a cause, not only a number

MedlinePlus lists possible causes that include testicular problems, liver or kidney disease, pituitary disorders, benign tumors, testicular injury, alcohol use disorder, and genetic conditions such as Klinefelter syndrome. FDA says approved testosterone products are for men with absent or low testosterone together with an associated medical condition.
