Boxed warning, serious skin reactions (SJS/TEN)
BOXED WARNING: Lamotrigine (Lamictal) can cause serious, sometimes fatal skin rashes, including Stevens-Johnson syndrome (SJS) and toxic epidermal necrolysis (TEN). The risk is highest in the first 2-8 weeks of treatment and increases significantly with faster-than- recommended dose increases, the starting dose and titration schedule below must never be exceeded. Stop lamotrigine immediately and seek emergency care at the first sign of rash, fever, swollen lymph nodes, or mouth/eye sores, unless your prescriber has told you the rash is clearly unrelated to the drug.
Mandatory slow-titration schedule (adult monotherapy, not taking valproate)
- Weeks 1-2: 25 mg once daily
- Weeks 3-4: 50 mg once daily
- Week 5 onward: increase by 25-50 mg/day every 1-2 weeks, per prescriber guidance, toward maintenance
If you take valproate (Depakote) or have missed doses for more than a few days, your titration schedule is different and slower, follow your prescriber's specific instructions, never a generic schedule.
