Boxed warning — serious skin reactions (SJS/TEN)
BOXED WARNING: Lamotrigine (Lamictal) can cause serious, sometimes fatal skin rashes, including Stevens-Johnson syndrome (SJS) and toxic epidermal necrolysis (TEN). The risk is highest in the first 2–8 weeks of treatment and increases significantly with faster-than- recommended dose increases — the starting dose and titration schedule below must never be exceeded. Stop lamotrigine immediately and seek emergency care at the first sign of rash, fever, swollen lymph nodes, or mouth/eye sores, unless your prescriber has told you the rash is clearly unrelated to the drug.
Mandatory slow-titration schedule (adult monotherapy, not taking valproate)
- Weeks 1–2: 25 mg once daily
- Weeks 3–4: 50 mg once daily
- Week 5 onward: increase by 25–50 mg/day every 1–2 weeks, per prescriber guidance, toward maintenance
If you take valproate (Depakote) or have missed doses for more than a few days, your titration schedule is different and slower — follow your prescriber's specific instructions, never a generic schedule.
